MAKE A COMPLAINT

Before you start the complaint form:

Please be informed that we shall only receive and examine complaints relating to a Ministry/Agency/Department (MDA) under Schedule 1 of the Public Service Act – Schedule 1 (Part 1: Ministries, Part 2: Government Agencies)

      • Before you contact us, you should first try to resolve your complaint with the MDA. If you are unhappy with the final outcome, then you may proceed to complain to us.
      • If you are a Public Servant, any grievance against an action/decision of a MDA, please refer to the Grievance Regulations (Public Service Grievance Regulation)

WHAT YOU NEED TO COMPLETE THE COMPLAINT FORM:

Before completing this complaint form, please ensure that

      • You have complained or attempted to resolve the matter with the MDA, however, you have not received a response within two  (2) weeks (please attach any documents relating to initial complaint)
      • You have any relevant documents/evidence required to support your complaint
